Other than a thourough cleanup, looks like you will need some sort of adjustable bottom vents, a charcoal basket and possibly a heat diffuser. Perhaps some short legs and a top vent cap too.

I would remove the crossbar, but leave 2" extended into the chamber. This will make it easier to get the charcoal basket in and out.

I'm thinking about cutting a door and installing a hinge.
Would I lose too much smoke that way?
I wouldn't cut a door. Cleaned up, a basket, and a new grate, thermometer, and you're in business!
No, you'll gain to much air. Unless you can seal off that bottom drain it wont burn like a drum.
I believe that is an old grease receptacle. You'll need to sandblast that down to bare metal and i'd still give it a good burn out.
