My butcher gets in only CAB packers with an average weight of 20 lbs.! Can I fit this on my 18.5 wsm? How appx. how long for low and slow?

It depends on how long the packer is... You may have to trim it to fit in the WSM. As for how long, a 20lber will take a long time low and slow...

In past post some have said to put something under the middle like a metal bowl or can raising it up so it does not touch the side. When it has cooked for awhile and shrank up a little you can take it out and finish it up or maybe put a little tinfoil loosely around the ends so it wont touch the sides.

General rule of thumb for cook time is approximately 1 1/2 hr per lb. but that depends on a lot of different things.
