yep, more times than i would like to admit. usually when i'm to lazy or forget to lay down some foil to catch the grease. it builds up and then catches fire when you have very hot coals.
no real biggy, just clean the grill more often. and i thought you had a serious problem. nice pics though.

Mr. Bigglesworth is correct. I've had the same thing happen on my OTS. Rinse it with a hose and then hit it with some simple green.

What charcoal are you using!?!? I only get one cook per load of charcoal on my OTS or OTG!

Something does seem out of place, that grill looks very clean for a well used grill. In 15 years you have never seen this before? You did not say what your last cook or cooks were which could have explained the fire. It does look like a common grease fire but I have also seen this happen with a sugar/honey drip, it will burn like crazy and puts off a nasty odor.

this happened to me when i was a beginner in bbq-i think like what bigabyte posted you
got a build up of fatty oils and was not aware of it,thats why its important to clean the smoker and grill after use. i use goggles and a steamer after i clean out the ashes a day later-you have to watch out for coals tht can still be active in the ashes-i have had coals still hot two day after i bbqed! if you cook fatty meats like lamb or ribs untrimmed like i do its important to clean everything also you should have a fire extinguisher near by the grill! i hope this helps http://thebarbecuemaster.net

Weird OTG fire...
Caked-on drippings from doing orange marmalade sriracha chicken wings can cause a fire like that, and in the ash catcher as well. I've had that happen to me a few times.
